Help expedite your visit by filling these forms out and bringing them with you along with your insurance card to your first visit, or you may fax them to (626) 628-0809

If you don't have access to a printer, please call (626) 628-0808 and we would be happy to mail you the forms in advance of your appointment

When you arrive please hand in your insurance card and filled out paperwork to the receptionist
